چکیده

A 28-year-old white female with a past medical history significant for idiopathic thrombocytopenic purpura and splenectomy in 1995 presented to an outside hospital after an episode of large volume hematemesis. She did not have any prior history of liver disease or any history of alcohol abuse. She underwent emergency endoscopy and 2 medium sized varices in the distal esophagus were treated with sclerotherapy. The patient was referred to our institution for further management. Upon arrival she had a CT scan of the abdomen that revealed previous splenectomy as well as a large gastric varix (Panel A arrow) and also demonstrated portal vein and superior mesenteric venous thrombosis with cavernous transformation. The thrombosis was felt be due to previous splenectomy and history oral contraceptive use. The patient recovered well and a follow up endoscopy 5 days later revealed small esophageal varices and the same gastric varix (Panel B) that was seen on the CT scan.

Practical Approach to Endoscopic Management for Bleeding Gastric Varices

Bleeding from gastric varices is generally more severe than bleeding from esophageal varices, although it occurs less frequently. Recently, new endoscopic treatment options and interventional radiological procedures have broadened the therapeutic armamentarium for gastric varices. This review provides an overview of the classification and pathophysiology of gastric varices, an introduction to c...

Gastric varices: Classification, endoscopic and ultrasonographic management

Gastric varices (GV) are responsible for 10-30% of all variceal hemorrhage. However, they tend to bleed more severely with higher mortality. Around 35-90% rebleed after spontaneous hemostasis. Approximately 50% of patients with cirrhosis of liver harbor gastroesophageal varices. In this review, new treatment modalities in the form of endoscopic treatment options and interventional radiological ...

Successful endoscopic submucosal dissection for early gastric cancer adjacent to gastric cardia varix

A 58-year-old man with liver cirrhosis and renal failure was diagnosed with esophageal varices (EVs) and a gastric cardia varix (GCV) by esophagogastroduodenoscopy (EGD). The patient also exhibited early gastric cancer (EGC) in the upper gastric body adjacent to the GCV. The EVs and GCV were treated using endoscopy before endoscopic submucosal dissection (ESD) of the EGC to prevent variceal ble...
